Forward membership includes access to in-person doctor visits at any Forward location as well as fully immersive virtual visits. You can choose from over 50+ expert Forward doctors soon after you join. You will also have 24/7 access to your Forward doctor and Care Team through the Forward App.
We have many more locations opening across the country in the coming year!
Your Forward membership covers in-person appointments with your doctor, 24/7 access to your doctor and care team via the Forward app, and in-office lab work. We can offer generic first-fill prescriptions onsite, vaccinations, genetic screening and counseling, and nutrition and wellness guidance. Forward becomes your health advocate ensuring you get the best care no matter what. This is all covered under your monthly membership fee, which means we don't bill your insurance for the services we provide and there are never hidden fees or co-pays with Forward. In-Office Lab Work Blood testing completed onsite during your Baseline visit includes your hemoglobin level to screen for anemia, a comprehensive metabolic panel to screen for kidney and liver function, a lipid panel to monitor cholesterol levels and optional STI screening. Results are ready in just 12 minutes for you and your doctor to review together in real time. Diagnostic testing for common illnesses like strep throat is also included in your membership.
Comprehensive Metabolic Panel This panel measures blood sugar levels, electrolyte and fluid balance, kidney function and liver function. The 14 measurements—potassium, bicarbonate, sodium, chloride, calcium, urea, glucose, creatinine, ALP, ALT, total bilirubin, AST, albumin and total protein—help provide a look at the overall health of your body and its metabolism and chemical balance.Your full cholesterol profile including HDL, LDL, total cholesterol, triglycerides and more. These data points help inform the general health of your heart, blood vessels and overall cardiovascular system.Because it’s better to know.
Lipid Panel Your full cholesterol profile including HDL, LDL, total cholesterol, triglycerides and more. These data points help inform the general health of your heart, blood vessels and overall cardiovascular system.
Complex Lab Work We can also facilitate complex lab work, such as a TSH or Testosterone test. These tests are processed offsite and will be billed through your insurance, separate from your membership fee.
Diagnostic Testing Quick and easy in-office testing for illnesses such as Rapid HIV, strep throat and mono.

Vaccinations We stand behind the medical and societal value of vaccines.We encourage our members to utilize the vaccinations included in membership as necessary. Seasonal and travel vaccines are also included in your membership.
Hepatitis A, B and Combo, HPV/Gardasil 9, Meningococcal, Pneumococcal 13 and 23, TDaP, Typhoid, Yellow Fever, Influenza, Influenza Intradermal, Varicella, Zoster, MMR, Cholera
Medications We have a pharmacy on-site for common generic medications like inhalers, antibiotics, ointments, and even blood pressure, and cholesterol medications. These items are included in the cost of membership typically for the first-fill. The membership does not cover any recurring medications or other medications that we don’t carry onsite. However, through our 24/7 Care Service, we can order prescriptions to your local pharmacy or have them delivered to your home.

We're a primary care provider with staff physicians trained in family or internal medicine, so our offerings are geared toward addressing many of your day-to-day needs, while also helping you avoid issues, identify problems early, and control chronic conditions before they become more expensive to treat. Membership also includes all consultations you have with Forward – at our locations, virtually, and in the app.
Services that are not part of our stated offerings are not included in your Forward membership.* These are things like:
Imaging (MRIs, x-rays, CAT scans)
Services requiring referral to a specialist (Cardiologist, Allergist)
Elective or planned hospitalizations
Urgent and emergency care, including hospitalizations
Prescriptions for ongoing needs and those not included in membership (that is, other than common generics kept in-clinic)
Ongoing treatment for severe depression, anxiety, substance use, and other mental health needs
Lab testing not included in membership (that is, other than common tests performed in-clinic)
*The scope of care is specified in your Patient Agreement, found at www.goforward.com/agreement. The Care Team is always available to answer your questions about which services are included in Forward membership.
The Care Team will always let you know if they recommend services not included in your membership and will work with you to anticipate and decrease out-of-pocket costs.

Forward is not health insurance and is not covered by it. Instead, we have one flat rate that covers all services offered by Forward. Forward does not accept health insurance because we believe health insurance companies are the root of our broken healthcare system. We’re working to rebuild the entire healthcare system from the ground up in order to fix it. We focus on delivering high quality, preventive, and personalized care that improves your health.
Meaningful, lasting changes in health and lifestyle take time. Because of that, your Forward membership begins with a six-month commitment. We believe this gives you, your doctor, and your Care Team the necessary time together to build a lasting relationship and provide you with the support you need to reach your health goals.
You may cancel your membership at any time after the six month period, but we are unable to refund you for payments made before the cancellation.
Refunds take 5-7 business days to be processed and reflected in your account. You're welcome to restart your membership at any time.

You will be charged for your first month of Forward on the day you sign up for your membership. You will then be charged monthly for your membership, on a go forward basis, on the same day each subsequent month. The monthly fee includes all services we provide. [See What isn’t included in my Forward membership?] Forward will never charge you a copay or send you an unexpected bill for a service we perform.
If a member needs additional tests or procedures that are not included in membership, then we will work to help the member try to find a non-Forward option that fits their needs. If the member has insurance, that typically means that we coordinate with their insurance provider to facilitate specialist referrals, prescriptions, or emergency care within their provider network.
Our membership model can help bring predictability to spending on primary care. It is, however, very important to understand that a Forward membership is not health insurance and is not a substitute for comprehensive health insurance. We encourage any person (including our members) who can afford it to maintain a separate comprehensive health insurance plan of their choosing.
Individual memberships are $149/month for all our sites.

Our referral program gives our members the opportunity to speak on Forward’s behalf to their friends and family.
We do have an affiliate program when it comes to referring members to Forward. We work with small groups and independent service providers who refer their clients to Forward in exchange for some type of credit. We’re only seeing new members over the age of 18.
Yes! You can sign up for the membership using your FSA or HSA card as you would any other credit card.
If you’d like to use your FSA or HSA card to pay for a portion of your purchase and apply the remaining balance to a different card, please reach out via chat or call us at 833-33-GOFWD. We’re happy to help set that up.

We offer certain generic medications onsite at Forward locations. First fills of medications for health conditions such as high blood pressure, high cholesterol, diabetes, asthma, allergies, flu, and issues requiring antibiotics (including z-packs) are included in the Forward membership.
Any prescriptions for medications not carried onsite or for recurring needs (beyond the first fill) are not included in your Forward membership. In these cases, you will be responsible for their cost. If you have insurance, this usually means having the prescription coordinated and billed through your insurance, especially when prior authorizations are required.
We’ll send the prescription to your local pharmacy, and can arrange for you to pick them up or for them to be delivered to you at home!
The Care Team will always let you know if they recommend services not included in the membership and will work with you to anticipate and decrease out-of-pocket costs in case you need additional services (e.g., specialists, hospitalizations, prescriptions, labs, imaging).
