Personalized, Preventive Healthcare

You will be charged for your first month of Forward on the day you sign up for your membership. You will then be charged monthly for your membership, on a go forward basis, on the same day each subsequent month. The monthly fee includes all services we provide. We're a primary care provider with staff physicians trained in family or internal medicine, so our offerings are geared toward addressing many of your day-to-day needs, while also helping you avoid issues, identify problems early, and control chronic conditions before they become more expensive to treat. Membership also includes all consultations you have with Forward – at our locations, virtually, and in the app. 

Services that are not part of our stated offerings are not included in your Forward membership.* These are things like:

• Imaging (MRIs, x-rays, CAT scans)

• Services requiring referral to a specialist (Cardiologist, Allergist)

• Elective or planned hospitalizations

• Urgent and emergency care, including hospitalizations

• Prescriptions for ongoing needs and those not included in membership (that is, other than common generics kept in-clinic)

• Ongoing treatment for severe depression, anxiety, substance use, and other mental health needs

• Lab testing not included in membership (that is, other than common tests performed in-clinic)

If you live in California, your Subscription also does not cover:

• More than thirty-six (36) in person or synchronous audio-visual telehealth visits with a licensed Forward healthcare provider (e.g., physician or nurse practitioner) during each twelve-month period of your Subscription; and

• More than thirty-six (36) in person or synchronous audio-visual telehealth encounters with an unlicensed Forward medical assistant during each twelve-month period of your Subscription (collectively, “California Excluded Services”, and together with Excluded Items, the “Excluded Services”).

Eligible HSA and FSA plans may be used to pay for Forward membership. Ultimately, it's up to your FSA or HSA provider to determine whether Forward's membership fee can be paid with your FSA/HSA funds. We're happy to provide documentation for you to submit to your FSA or HSA provider. Please reach out via chat or call us at 833-33-GOFWD for assistance.

We believe this gives you, your doctor, and your Care Team the necessary time together to build a lasting relationship and provide you with the support you need to reach your health goals.

We’ve designed Forward to give you access to more information so you can feel in control of your health. That approach extends to include control over your personal health information.

We do not sell your health data, and we implement tools like encryption to safeguard your privacy.

Forward does not accept health insurance. Instead, we have a flat rate that covers all services offered by Forward – there are NO copays or deductibles.

Our flat rate keeps billing simple and empowers members to fully utilize our services – including in-person and remote visits, in-house labs, 24/7 chat, and personalized health programs.

Your monthly membership can be paid for with an FSA or HSA.

Like other forms of insurance, Medicare and Medicaid do not cover Forward’s membership fee. Members pay one flat rate that covers all services offered by Forward and Forward does not seek reimbursement from Medicare or Medicaid.

Forward’s preventive approach meets most day-to-day primary care needs and gives you consultations for a healthy future. As a Medicaid member (except in Colorado) or Medicare member you can enjoy these services at the same flat rate that all other Forward members pay.

In the event you need services that are not included in your Forward membership, we work to help you manage your out-of-pocket costs.

Yes absolutely. Insurance is not required to become a Forward member. However, it is important to understand that Forward membership is not health insurance and is not a substitute for comprehensive health insurance. Whether you have insurance or not, you will pay the same flat fee for your care at Forward.

Forward is not health insurance and therefore does not fulfill the ACA’s health insurance requirement.
