* Forward can perform the sample collection for most lab tests, but any tests sent to an external lab for processing may incur additional costs that will be billed by the lab to your insurance or directly to you if you do not have insurance. Our onsite team will notify you whenever this occurs.
At the beginning of your membership, you will select your primary care doctor. Together you and your doctor complete your Baseline visit to establish your goals and personalized plan. Throughout the membership, you’ll have an opportunity to build a relationship with your doctor through shared decision-making based on trust and transparency.
We employ doctors from top universities like Harvard, Stanford, and NYU, having completed their residencies at institutions like Cedars Sinai, Mt. Sinai, and Columbia University.
You’re always able to get support from other Forward physicians, whether you’re traveling around the country or simply want a second opinion. You’re also able to communicate with other physicians and nurse practitioners through the messaging feature available 24/7 in the Forward app.
While we don't currently provide imaging services, your Forward doctor is able to order specialist services like imaging, when necessary. Imaging is performed outside of Forward, and so is not included as part of your Membership. In those cases, you are responsible for paying the outside specialist. If you have health insurance, for instance, the service is billed through that insurance.
Forward continues to add more services over time giving members more value.
In the event you need services that are not currently included in your Forward membership, we work to manage your out-of-pocket costs. If a member has insurance, we coordinate with their insurance provider to facilitate specialist referrals, prescriptions, or emergency care within their provider network if needed. Savings can be compounded by paying for Forward membership using a tax-advantaged mechanism like an FSA or HSA.
Forward membership includes seamless prescriptions ordering, refill management and vaccines. We have a pharmacy at our locations for common generic medications like inhalers, antibiotics, ointments, and even blood pressure, and cholesterol medications. However, the membership does not cover any recurring medications or other medications that we don’t carry onsite. Our 24/7 Care Team and team of doctors can order prescriptions to be delivered straight to your home or have you pick them up at your preferred local pharmacy. We also take over your prescriptions from your existing providers so you don't have to worry about starting again. Finally, we also include preventive vaccinations such as flu, COVID-19, HPV and other standard vaccines all at no additional cost.
Regular pap smears are an important preventive screening that we provide at all our Forward locations. Our doctors will review your gynecological health in detail with you and make personalized recommendations regarding the frequency of pap smear testing that is right for you.
Forward doctors can perform the pap smear, but due to local and state regulations, Forward is unable to pay for the lab testing portion of pap smear examination on behalf of our members.
If you have insurance, the lab completing the test will bill your insurance directly. Most insurance plans are required to cover the full cost of the pap smear, which means you should not be charged anything (except a copay where applicable). If you are uninsured, the laboratory performing the pap smear will bill you the cost for the pap smear directly.

In the traditional healthcare system, lots of people feel like they keep paying more and getting less. Healthcare today is often expensive, reactive, bureaucratic, and frustrating. At Forward, we shared that frustration with the healthcare industry’s unresponsiveness to such widespread dissatisfaction and poor outcomes. So we set out to build something different: an entirely new model of primary care, designed for people.
Forward memberships are $149/month. This one flat monthly fee gives you unlimited access to Forward services. Forward will never charge you a copay or send you an unexpected bill.
You will be charged for your first month of Forward on the day you sign up for your membership. You will then be charged monthly for your membership, on a go forward basis, on the same day each subsequent month. The monthly fee includes all services we provide. If a member needs additional tests or procedures that are not included in membership, then we will work to help the member try to find a non-Forward option that fits their needs. If the member has insurance, that typically means that we coordinate with their insurance provider to facilitate specialist referrals, prescriptions, or emergency care within their provider network.
Our membership model can help bring predictability to spending on primary care. It is, however, very important to understand that a Forward membership is not health insurance and is not a substitute for comprehensive health insurance. We encourage any person (including our members) who can afford it to maintain a separate comprehensive health insurance plan of their choosing.
We're a primary care provider with staff physicians trained in family or internal medicine, so our offerings are geared toward addressing many of your day-to-day needs, while also helping you avoid issues, identify problems early, and control chronic conditions before they become more expensive to treat. Membership also includes all consultations you have with Forward – at our locations, virtually, and in the app.
Services that are not part of our stated offerings are not included in your Forward membership.* These are things like:
Imaging (MRIs, x-rays, CAT scans)
Services requiring referral to a specialist (Cardiologist, Allergist)
Elective or planned hospitalizations
Urgent and emergency care, including hospitalizations
Prescriptions for ongoing needs and those not included in membership (that is, other than common generics kept in-clinic)
Ongoing treatment for severe depression, anxiety, substance use, and other mental health needs
Lab testing not included in membership (that is, other than common tests performed in-clinic)
*The scope of care is specified in your Patient Agreement, found at www.goforward.com/agreement. The Care Team is always available to answer your questions about which services are included in Forward membership.
The Care Team will always let you know if they recommend services not included in your membership and will work with you to anticipate and decrease out-of-pocket costs.

Like other forms of insurance, Medicare and Medicaid do not cover Forward’s membership fee. Members pay one flat rate that covers all services offered by Forward and Forward does not seek reimbursement from Medicare or Medicaid.
Forward’s preventive approach meets most day-to-day primary care needs and gives you consultations for a healthy future. As a Medicaid member (except in Colorado) or Medicare member you can enjoy these services at the same flat rate that all other Forward members pay.

From the moment I came in it was personal attention with no interruptions. I have never enjoyed going to the doctor like I did here. You come in and first thing they do is take your vitals using state of the art technology. Once that is done you get taken into your patient room where they draw blood and give you snacks and drinks before the doctor comes in.
My doctor, Dr. Chesda was super nice and gave me so much time to go over health questions and concerns I had and thoroughly went over all my test results which were available immediately on a huge touch screen. We covered so much in one sit down and set goals with my doctor to improve on my health.
Now I have all my results to view on my secure app and I can also chat with my doctor or any team member if I have any questions or concerns. What? My healthcare provider available 24/7 to answer all my questions and concerns? Super amazing! Just found my new healthcare provider for life.

I anticipated this from the Cali reviews and saw the overwhelmingly positive responses and so I waited (I survived before Forward with the providers I had and knew it would be good). MOST IMPORTANTLY: coming from healthcare myself, I knew that the initial 100-120-minute is a big block of time that needs to be scheduled later in the game of appt "Tetris" because current members fill up faster with their small appts. Spoilers: I get appts pretty easily at any of the 2 NY spots - 4 providers.
In my first visit in December 2018, I was blown away - like everyone else was. This is usually where most reviews end.
A ton of excitement at how awesome the docs are and impressed by the speed, accuracy and attention to detail of the first visit; so I will keep this part short. I was incredibly impressed by Dr. Singh. I work at the top 4 NY metro area hospitals with some the best nationally and internationally renown doctors in the N-E region. I get the same feels from all of 4 of the NY Forward docs, but more on that later. I'm not easy to impress. Singh impressed me with her thoughtful explanations and approach to my baseline visit. The real gem was how she didn't want to just throw antibiotics at me (as most traditional PCPs do) but instead gave me a plethora of ayurveda homeopathic ways to help my sinuses and ENT probs. I was viewed holistically and was shown the connections of related issues in my care plans. The blood work, the blood draw, the snack, change of clothes was great. The best part- everything was captured on screen and sent to my care plans in my app later in the day and update/add on appropriately with subsequent visits. The body mapping was accurate. It was more accurate than other places I went to for advanced morphometrics with messed up machines. Forward's measurements coincided with the same measurements elsewhere - so trust the accuracy (you don't have to waste $ elsewhere): their body scan is top notch. My visit was amazing just like (nearly) all the other reviews.
Genetics: I was lucky enough to be offered the kit at the first visit. I knew from Forward reviews in Cali that sometimes it gets offered months later. I'm sure it is a logistical out-of-stock issue and being patient pays off. That visit was an eye-opener and game changer. The results coincided perfectly with my family history and pharmacogenetic testings I did for my parents with another company. I have the rare, unique ability (because of testing parents and Forward testing me): to know where things came from but more importantly exactly what SNPs I have with quantified associated increase/decrease risk to the general population. Not a gimmick in any way and very real. Singh did an excellent job delivering so much info to me and answering my Qs at a later appt.
The rest - I experienced it all. Needed to go to Europe and got sick: I was able to get a same day appt and fly out later on! That was a hell of a day but that stylish goodie bag Dr. Kennedy gave me saved me (DayQuil, painkillers, emergen-C, lozenges). That was one of the most thoughtful gesture ever. You would never get that from a reg PCP! I get appts easily and the care team reaches out to me for appt offers that better fits my schedule when I indicate another time and day works best with my busy schedule!
Records - They managed to get almost all of the records from previous places (5/6) - with the 1/6 being a major hospital with a stubborn outdated personnel who refuse to request through 3rd party EHR requests companies in order to not lose their jobs at patient records dept. There simply are places where YOU (not Forward, disclosure policies I'm familiar with) will have to call, get mailed a disclosure form to sign to release your PHI (protected info). Devices - I wasn't really in it for devices and so I can't speak on monitors. THE CHAT/THE APP - SO my experience with the phone app is superb - no surprise knowing iPhone dominates the app market and there is only one iOS so... Of course Android users get glitches, as per the many permutations on the Android PHONE market: same reason for issues on almost every app in the play store.
As an uninsured freelancer in healthcare: I know the value of getting a dx through pic/text chat without having to be physically seen is a MAJOR value: Ophthalmic probs, ENT probs, aches. Their 24/7 chat service with amazing APNs is a GREAT value. More than justifies the cost with saving time and $. I was even fortunate enough to get Rx'd a med by Dr. Dufresne through the app; I would have been screwed that weekend. Office experiences have been great. My PCP, Dr. Graziano, is brilliant. Oozes so much intellect. How awesome is it that I get treated by a brilliant Ivy-league doc who helps me understand (and actually learn) about the body in such a comprehensive way. She is so down to Earth and helps me throughly understand the cons of modern wellness trends, redirects me to proven solutions and welcomes me back for unlimited testing; I'm still waiting for her to move objects with her mind though. They don't seem to have small 15 min appts - every appt is 30 mins. Even on 30 mins appts for something as small as getting a shot - Graziano managed to bring up topics on my care plan and make the most of my time (when I thought she would take advantage and keep it short and grab lunch): I appreciated the amazing clarity she brings to the table (which many traditional PCPs lack) and thoughtfulness. I have also received post-visit PDF plans akin to PHYSICAL THERAPY plans: an issue most hard-working NYers, in some way, could benefit from. Never got illustrated detailed PDFs (not even a paper) from previous regular PCPs before! The docs look young but their work history at hard-to-get-into prestigious institution insures they seen it all, seen all ages and worked with the best resources and mentors. One of them was even an intensive care unit (ICU) doc - outstanding credentials, all of them!
Long review but I can say a lot and evaluate more because of my experience in HC, with other companies and 7 months-Forward. The exciting part is they will add services - the Heart Ultrasound, the dermatology scan and the genetics updates with discoveries contingent on STRONG clinical evidence. Other companies have their genetic services show you everything with varying levels of certainty: resulting in things other docs will ignore as unproven (seen it at work) or patients worrying over nothing. The docs, the APNs, the CMAs, admin and developers have really made it amazing. I'm not the only one who thinks so either. Ty for reading this. Good luck.

The staff are all so friendly. For the first time in my life I didn't mind having blood drawn. I think We all just get used to being treated like a number by the medical field. Now I feel like a person who matters Such a great feeling.

During this first encounter, I learned that basic blood tests are included in my membership, but certain more targeted tests, which pertain to my particular condition, are LESS expensive than my current HMO was charging me! Also, the doctor gave me information about how to obtain my ongoing prescriptions for a fraction of what I have been paying, effectively canceling the cost of my membership. Amazing!

All the notes, results, and action items were helpfully summarize in my app, and the Forward team is FAST in dealing with referrals, record requests, and other items doctors offices typically take ages to take care of. I was skeptical, but this really does fee like how healthcare should be.

As others have mentioned, the baseline exam is really thorough and lab results are available almost immediately. Thankfully everything was within range so I didn't need to walk out with much but they made sure I didn't have any open questions on the teatnus booster, and I didn't feel rushed at all.
I think the combination of a plan here and a high deductible plan at work still reduces my overall health care premium each month vs. taking a higher level insurance plan and I'm looking forward to having this new concierge service available 24/7.

What was most impressive was the level of follow up via messages through the app. Follow up was exponentially better than with any doctor experience in the past.
Overall I would say that Forward is a great option and for me in particular a fantastic supplement (not replacement) to my existing insurance and medical regimen.
