These promotional terms and conditions ("Promo Terms") apply to the purchase and use of Forward’s Advanced Diagnostics product ("Product") offered by Forward ("we", "us", or "our"). By purchasing the Product, you ("you", or "your") agree to be bound by these Promo Terms. Please read them carefully. The Product is also subject to terms and conditions of Forward’s membership agreements, which you will be asked to consent to as part of your purchase.
The Product is available for purchase and use only during the promotional period starting from 7/15/24 and ending on 9/2/24. ("Access Period"). The opportunity to purchase and/or use the Product outside of this timeframe is not guaranteed and is at the sole discretion of the Company.
This promotion is open only to individuals aged 18 years or older at the time of purchase. The Product is available only while supplies last and is subject to availability.
The Product is non-transferable. Purchase of the Product is limited to you and cannot be shared or transferred to any other person.
All sales are final. We do not offer refunds or credits for purchases made, including for any unused portion of the Product after the Access Period ends.
A portion of the Product may be provided by one or more third parties and thus subject to their additional terms. If you select the full-body MRI option, the MRI will be provided by Ezra; see Ezra’s terms and conditions and FAQ for details.
The patient-provider relationship established through the purchase and use of the Product is strictly limited to the services expressly included as part of the Product: specified laboratory tests (if selected by you), a full-body MRI (if selected by you), clinical review of the results of such laboratory tests and/or MRI, as applicable, a report reflecting all such results, and a 45-minute visit with a Forward physician to review such results. The patient-provider relationship established pursuant to the use of the Product will automatically expire and all obligations and responsibilities of the provider to you will cease upon the termination of the Access Period. You are encouraged to consider your healthcare needs beyond the duration of the Access Period and to plan accordingly for your ongoing healthcare requirements.
The expiration of the patient-provider relationship does not affect your right to access your medical records as per the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and other applicable laws and regulations. See Forward’s Notice of Privacy Practices for more information.
We reserve the right, at our sole discretion, to modify or replace these Promo Terms at any time. By continuing to access or use the Product after those revisions become effective, you agree to be bound by the revised terms. Promo Terms void where prohibited.
